Above are a few link's to free services that will provide an EMS wallet card.   If you are so inclined - fill out and  print.  Laminate if preferred and put in wallet or someplace convenient such as with drivers license or insurance card.

Fill this card out as accurately as possible.  Now is not the time to be confidential with any health problems.  In the event someone has to look at this card its usually because there has been some sort of an emergency or accident and they will need to know specific details of your health issues and any medications you might be taking.   I understand some people are private or don't like to divulge their health issues - but in this case it may be a life and death decision made by what you have listed on this card. 

It is also important to keep this card updated as your health changes.  Medications started or stopped etc.  There is no limit to how many cards you can print so go back and change your information if needed and re-print.
